An Internal Medicine Physician at AbsoluteCare provides comprehensive healthcare to medically complex and chronically ill patients, focusing on managing their conditions to reduce hospital visits and improve quality of life. They lead multidisciplinary teams, coordinate transitional care, and treat acute exacerbations within a patient-centered medical home model. The role emphasizes evidence-based medicine, community engagement, and addressing barriers to healthcare for vulnerable populations.
