We are hiring PRN Registered Nurses for Home Care here at Charlestown Retirement Community.

This position requires experience utilizing the Oasis charting system and a minimum of 1 year of recent home health experience .

PRN Compensation: $48.00 per hour. Potential for $2.00 per hour shift differential for evening and night shifts.

How you will make an impact:

• Partnering and collaborating in the initial evaluation to assess the residents needs to develop and implement a service plan based on the evaluation and in conjunction with the resident, family, physician, and /or authorized practitioner
• Communicating any changes in the residents condition
• Ensuring that the residents individualized plan is implemented and followed by the Home Support agency team of providers
• Delivering nursing care as outlined in the plan of care and according to acceptable nursing standards
• Participating in care conferences and other residentrelated meetings
• Participating in the orientation and training of other agency nurses as well as training any Aide staff development activities

PRN REQUIREMENTS:

• To complete community and department New Employee Orientation and Department specific orientation.
• To complete department orientation shifts with 45 days of hire.
• Must be available to train on weekdays during orientation period.
• To work, at minimum, one weekday and one weekend (consecutive Saturday and Sunday, minimum 4 hours per day) per month

• To work one winter and one summer holiday per year (New Years Day, MLK Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas)

What you will need:

• Current Registered Nurse license for the state in which they practice
• Recent home care experience required
• OASIS experience required.

The Home Care RN provides nursing care according to established policies and procedures for a home care agency. The RN develops and supervises the implementation of a residents individualized service plan, observes the residents condition, and communicates to the Physician and family/responsible party as needed. The RN oversees the residents care delivered by the Home Support team of LPNs, Aides, and Home Care workers and companions.
